Application for Borrowing Privileges at Northwestern

This form is for eligible University of Chicago users who would like to apply for borrowing privileges at Northwestern's participating libraries.

Please note: You can get a card more quickly by going directly to the Library Privileges office at Northwestern Library between 8:30 am and 5:00 pm Monday through Friday. Using this form means you will have to wait 5-7 working days before your card is available. Please complete all the fields listed below.
