Library Change of Address Form

Please use this form to notify the Library in the event of a change of address. When you borrow from the Library, you assume responsibility for keeping a current address on file. If you do not receive a Library notice because you have not provided your change of address, you will be held responsible for resulting fines.

PIN:	For students, the Personal Identification Number (PIN) is your student identification number. Faculty, staff, and other library card holders should request a PIN online or see a Circulation supervisor.
