This form should be used only by alumni of the University of Chicago and users not affiliated with the University of Chicago.

Complete this form in the event of a change of address. When you borrow from the Library, you assume responsibility for keeping a current email and mailing address on file. If you do not receive a Library notice because you have not provided your change of address, you will be held responsible for resulting fines.
