Appeal a Fine or Claim a Return

Complete this form to appeal a fine on your account, to claim that an item should not be on your account, or to report a lost or damaged item. In all cases, please provide as many details as possible. If you need to resolve a matter related to your account immediately, please contact a Circulation Supervisor.

This form should be used for Library items only. If you have borrowed equipment from the IT Services TECHB@R, AVServices in Stuart Hall, or the Logan Media Center, please contact staff at those locations directly.

A reminder: if you borrow items for another person on your account, you are responsible for those items. More information about due dates, fines, and appeals can be found under Borrowing Policies.
