Social Service Administration Circulation | Appeal a Fine or Claim a Return

Please complete this form to appeal a fine on your account, to claim that you returned an item that is still on your account, or to claim that an item on your account isn't yours. If you need to resolve a matter on your account immediately, please contact a Circulation Supervisor.

If the item is claimed returned, Library staff will search for the item for six weeks, or longer if necessary. During this time, the item will remain on your account. If the item is found, it will be removed from your account. If the item is not found, you are accountable for the item and may be billed for the costs of replacement and processing. If your initial appeal is denied, you may file another appeal with a library officer.

You must complete a separate form for each item. Fields displayed with a * are required.

Claim never checked out There is an item on your account that you believe you never checked out. If you check out items for another person, you are responsible for those items.
Appeal a fine If you feel that a fine has been inappropriately applied to your account.
