Appeal a Fine or Claim a Return

Please complete this form to appeal a fine on your account, to claim that you returned an item that is still on your account, or to claim that you never checked out an item listed on your account. In all cases, please be as specific as possible and provide as many details as you can. If you need to resolve a matter on your account immediately, please contact a Circulation Supervisor.

Appeal or Claim Type (select one)

Claim return of an item There is an item on your account that you believe has been returned.
Claim never checked out There is an item on your account that you believe you never checked out. If you borrow items for another person on your account, you are responsible for those items.
Appeal a fine If you feel that a fine has been inappropriately applied to your account.
Other You have lost an item checked out to you or an item is damaged while checked out to you.
